Physical Therapist (PT) – Home Health

Position Overview

Join our client’s dedicated Home Health team as a Physical Therapist, where you’ll make a meaningful difference by helping patients achieve greater independence and improved quality of life—all from the comfort of their own homes. This role offers a supportive clinical environment, a predictable schedule, and the opportunity to build strong relationships with patients and caregivers.

Required Qualifications

  • Master’s degree or higher in Physical Therapy
  • Active and unrestricted Physical Therapist (PT) license in the State of Illinois
  • Minimum of 1 year of experience in a healthcare setting
  • Valid driver’s license and ability to travel locally

Preferred Qualifications

  • PT license number provided at time of application

Key Responsibilities

  • Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
  • Provide in-home therapy services focused on improving physical and cognitive functioning
  • Monitor, document, and report patient responses to treatment and changes in condition
  • Educate patients and caregivers on therapeutic exercises, safety, and use of adaptive equipment
  • Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants
  • Complete required documentation, prepare clinical reports, and participate in team and agency development activities

Schedule & Travel

  • Monday–Friday | Day shift (start and end times vary based on patient needs)
  • One weekend every six weeks (Saturday & Sunday, scheduled visits only)
  • Two holidays per year (one summer, one winter on a rotating basis)
  • No on-call requirements
  • Local travel to residential homes
